Health Services

According to state guidelines, students have 30 days from the date of enrollment to have all required immunizations or be in the process of getting the immunizations or acquire an exemption from the state. The Benton County Department of Health offers the required immunizations. Its phone number is 479-986-1300.
State of Arkansas Immunization Requirements (effective Sept. 1, 2014) All students Kindergarten through 12th Grade must have the following:

• 4 doses of DTaP with one dose after 4th birthday
• 3 or 4 doses of polio with one dose after 4th birthday
• 2 doses of Varicella (chickenpox)
  • May accept written documentation of history of chickenpox disease from
  • M.D. /D.O. /A.P.N. /P.A. only.  Can no longer accept parental history of disease
• 2 doses of MMR
• 3 doses of Hepatitis B

 Additionally:

• If your child is entering Kindergarten they must have:
  • 1 dose of Hepatitis A
• If your child is entering 1st Grade they must have:
  • 1 dose of Hepatitis A
• If your child will be age 11 or older on or before September 1 they must have:
  • 1 dose of Tdap
• If your child will be entering 7th Grade they must have:
  • 1 dose of Meningococcal Meningitis
• If your child will be age 16 on or before September 1 they must have:
  • 1 or 2 doses of Meningococcal Meningitis (only one dose required if 1st dose is received on or after age 16)           

The school district will assist parents in administering medication to students if indicated during the school day. Parents are encouraged to administer medicine at home before school, after school and at bedtime.

All medications (prescription and nonprescription) must be delivered to school personnel by the parent/guardian.  Medications will not be transported on the school bus.

All medications must be in the original container and must be accompanied by a medication permit filled out by the parent or guardian.

Kindergarten Physicals
Comphrehensive and Developmental Preschool Examination:  All enrolling kindergarten students shall be evaluated with Periodic, Screening, Diagnosis and Treatment (EPSDT) equivalent.

A kindergarten student should have a physical screening within the two years prior to or up to 90 days after the student's initial enrollment in kindergarten.  Vision All children in pre-kindergarten, kindergarten, grades 1,2,4, 6, 8 and all transfer students will receive vision screening. Any student referred will also receive vision screening. A child who does not pass the screening must be seen by an optometrist or ophthalmologist within 60 days of receipt of the screening report notifying the parent of the failure.

Each school in the Rogers School District has at least one AED available.

Dear Parent/Guardian:  It is very important for you to notify the school when your contact information (telephone number, address, etc.) changes. The school should have an up-to-date contact number in the event your child is ill, an emergency situation occurs or other important information needs to be conveyed to you about your student's education.
